A randomised controlled study for comparison of primary closure of common bile duct versus t-tube drainage in case of choledocholithiasis

چکیده
The closure of common bile duct (CBD) after choledocholithotomy can be done over T -tube or primarily. The method of T-tube drainage comes with many complications and hence, nowadays, there is tendency to close CBD primarily which leads to less complications and shorter duration of stay at hospital. We conducted this study comparing primary closure of CBD with T-tube drainage following choledocholithotomy over a period of one year in 40 patients divided in two groups each of 20 i.e. Group A – primary closure and Group BT-tube drainage . The results were compared in terms of operating time, duration of stay at hospital and complications such as leakage and wound infection. It was observed that primary closure group had less operating time, less duration of hospital stay and less complication rates as compared to T-tube group. Hence, we concluded in our study that primary closure can be recommended as safe alternative procedure after choledocholithotomy in selected patients of choledocholithiasis.

Spontaneously removed biliary stent drainage versus T-tube drainage after laparoscopic common bile duct exploration
Several studies have shown the safety and feasibility of laparoscopic common bile duct exploration (LCBDE) as a minimally invasive treatment options for choledocholithiasis. Use of T-tube or biliary stent drainage tube placement after laparoscopic choledochotomy for common bile duct (CBD) stones is still under debate. This study tried to confirm the safety of spontaneously removable biliary ste...
متن کاملComparison of Primary Closure and T-tube Drainage following Laparoscopic CBD Exploration
Purpose: Laparoscopic common bile duct exploration (LCBDE) has traditionally been accompanied by T-tube drainage. However, patients must carry it for several weeks and often suffer problems related to the T-tube. So, primary closure of CBD has been proposed as a safe and effective alternative to T-tube placement after laparoscopic choledochotomy. The aim of this study was to compare primary clo...

BACKGROUND This study aimed to assess the clinical short-term results of a primary closure following laparoscopic common bile duct exploration (LCBDE) combined with intraoperative choledochoscopy and D-J tube drainage for choledocholithiasis treatment. MATERIAL AND METHODS Twenty-five patients (14 women and 11 men) who underwent LCBDE with primary duct closure and D-J tube drainage for choledoc...

Introduction:Even with the emergence of laparoscopic common bile duct exploration, open common bile duct exploration remains the procedure of choice for the surgical management of choledocholithiasis in developing countries. Open CBD exploration is usually followed by a T-tube insertion whichhas been proven to be safe and effective for post-operative biliary decompression. However, the use of T...
متن کاملRisk Factors Associated With Residual Stones in Common Bile Duct Via T Tube Cholangiography After Common Bile Duct Exploration
Open surgery with common bile duct (CBD) exploration and T tube drainage are still traditionally performed in a large amount of selected patients with cholelithiasis and choledocholithiasis. Confirmation of CBD clearance via T tube cholangiography after surgery is a routine procedure before T tube removal. The present study aims at investigating potential risk factors associated with residual s...
